Educational Assessment Checklist

We are striving for 100% participation in completion of this checklist by all advocates for all children attending pre-school or higher. If a child may have special needs which will impact their future school programs, there are services for children who are infants as well. If this may be the case for your CASA child, we would appreciate your completion of this checklist. You will find the online checklist form at the bottom of this page. There is a second half of this checklist that is much more in-depth, for those who wish to become more involved in gathering educational information for their CASA child(ren) or youth. This can be dowloaded in Word at Educational Assessment Checklist Part 2

After you have completed part one of the Educational Assessment Checklist, we hope you will meet with Steve, Yvonne, and/or your case supervisor to review the educational strengths and challenges of your CASA child(ren) or youth for the second phase of the project. From this meeting, we hope to develop clearly structured action plans in support of at least one of 7 measurable objectives. The implementation of these plans is the third phase of this project.
