Transitions occur when ever a child changes programs, school settings or service providers. Part of the IEP team's responsibility is to make plans so that transitions occur smoothly and with the least disruption of services needed for a child; based on the needs of the particular child, considering physical, social and emotional health. For children over 14, transition plans should address the child’s goals beyond graduation and show how planned studies and services will help the child attain these goals. Laura Madjedi will discuss examples of Transition Plans and what role advocates can play in this process.
